28 * Associate and Young Professional initiation fee is paid over three years. QUALIFICATION FOR MEMBERSHIP (Numbers refer to classifications above)
1. Family membership includes spouse and children under the age of 19 years. Children 19-24 years, see #5 Intermediate. 2. Single memberships consist of a single, unmarried person. Single members must transfer upon marriage to a Family Athletic membership within 60 days. Proof of marriage required. 3. Senior members shall consist of applicants 72 years of age and older. In the event of death of a Senior Family member, the surviving spouse under 72 years of age may maintain the Senior status membership. If a surviving spouse under 72 years of age remarries, the membership shall be reinstated to Family Athletic status and dues. 4. To qualify for Intermediate membership, the applicant’s par ents must have an active Family Athletic membership. The applicant must be single, living at home or going to college. Upon marriage, an Intermediate member has the option to terminate membership or convert prematurely to Associate membership. 5. To qualify for Associate membership, applicant’s parents must have an active Family Athletic membership. The applicant pays the initiation fee in three payments: $800 upon application and two consecutive yearly payments of $850 each. At age 30, the Associate member must convert to Family Athletic or Single Athletic member dues. There is no additional initiation fee with this transfer.

Social membership does not include children. All athletic facilities and programs are restricted from use by a Social member. 8. Corporate Athletic and Corporate Additional Designate Athletic Memberships are owned by the applicant business firm. The firm may designate an employee as a member for each Corporate athletic and additional designate membership application. The designate and designate’s family (spouse and dependent children under 19) will be allowed full membership privileges. Dependent children ages 19-24 may remain on membership by paying Intermediate dues. When a designate of the membership is no longer employed by the business he/she no longer has membership privileges at the Bellevue Club. It is the responsibility of the business firm to notify the Club to cancel the designation. The membership is not transferable to a new company. A Corporate membership may be reassigned to another employee within the company by the business firm submitting a written notice to the Club and payment of $150 transfer fee. However, a Corporate membership may not be terminated when there are Additional Designates under the membership. The company
will be held responsible for any delinquent or outstanding dues/charges on Corporate membership accounts. 9. A business firm may designate an employee (to include spouse, if requested) to use the social facilities of the Club. The Professional designate will not be allowed to use any athletic facilities of the Club. Corporate Professional designates may not be transferred. When a designate ceases to be employed by the firm, he or she no longer has any membership privileges. 10. To become Inactive, a member must be relocating residence be yond a 100-mile ra dius from the Club for a minimum of one year. Club retains mem ber ship card(s) during the inactive status and member may not use the Club facilities during inactive period.
