PURPOSE: To establish a dress code that is business-related and professional and to provide a safe, secure environment for patients and employees Employees contribute to the culture and reputation of CRHC in the way they present themselves. A professional appearance is essential to a favorable impression with patients, regulators, and other visitors. Good grooming and appropriate dress reflect employee pride and inspire confidence on the part of such persons. PROCEDURE: 1. Each manager or department head is responsible for establishing a reasonable dress code appropriate to the job the employee performs and their assigned location. 2. Employees are responsible for inquiring as to the appropriate dress code for their department and for following those guidelines. 3. All Employees who work in clinical areas are required to use the personal protective equipment provided by the Practice in accordance with OSHA regulations. Failure to do so is subject to disciplinary action up to and including termination. 4. Employees are required to wear their ID badges. Name badges must be visible. The badge should be worn on the left lapel area. 5. Minimum Dress Requirements include:  No jeans (blue denim/dungaree material)  No tank tops, spaghetti straps, tube tops, halters, low scoop necklines  No bare midriffs  No shorts or tee shirts (undershirts)  No shirts/sweatshirts with writing/advertising clothing made of spandex material

Pierced earrings are permitted to be worn only when worn in ears. Other exposed piercings are not permitted (tongues, eyebrows, etc.). Discreet, clear piercing retainers are allowed. Simple jewelry Employees wearing white uniforms should wear appropriate full-cut white undergarments (no printed, colored, bikini or thong underwear)

6. The following is a breakdown of the dress code for specific areas: Administrative/Clerical-Office Personnel Appropriate business dress is required which includes dresses, suits, slacks, skirts, jacket and tie, if appropriate. Clinical Personnel Leather shoes/sneakers Printed/Solid smock Solid pants/skirts Lab Coats (when appropriate) Dangling chains, earrings or bracelets are not appropriate and maybe a safety hazard to patients and staff. Fragrance Sensitivity Crystal Run Healthcare strives to ensure the comfort and safety of employees, patients and visitors. In order to accommodate co-workers, patients and visitors of the Practice who are chemically sensitive to fragrances and other scented products, CRHC requests that employees refrain from using air fresheners, scented cleaning agents, perfumes, colognes, aftershaves, potpourri, scented candles, scented oils, scented lotions or hair care products in the work environment. 7. Addressing Workplace Attire and Hygiene Problems Violations of the policy can range from inappropriate clothing items to offensive perfumes and body odor. If a staff member comes to work in inappropriate dress, the staff member will be required to go home, change and return to work. If a staff member’s poor hygiene or use of too much perfume/cologne is an issue, the supervisor or human resources will discuss the problem with the staff member in private and will point out the specific areas to be corrected. If the problem persists, supervisors will follow the normal corrective action process.
