FNP Practicum Application Guide Submitting your practicum application does not guarantee registration. Registration is based on multiple approval processes. Once you have been approved, you will receive a separate email confirming your enrollment in your practicum courses.
Before You Begin - Review the MSN FNP Practicum Handbook You can view the MSN FNP Practicum Handbook by visiting chamberlain.edu/handbook. Please read through the practicum handbook in its entirety to ensure you are fully prepared to submit the practicum application and any other required documentation. Failure to comply with all necessary requirements may delay enrollment in the practicum and postpone your graduation.
Step 1: Student Portal Log-in Directions To access your student portal, please visit my.chamberlain.edu. You can log in to the portal using your Student ID (D#) and password. If you have forgotten your D#, please call the Student Services team at 888.556.8226. For your first time logging in, your default password will be your date of birth in the Mmm-YYYY format, (e.g., Mar1980).
Step 2: Submit your Practicum Application To begin your application, you will need to create a case through the student portal. 1. From the student portal’s home page, click on the Resources link in the left menu.

FNP Practicum Application Guide Step 2: Submit your Practicum Application (Continued) 6. Under the Department drop-down menu, select Student Finance/Academic Advising. Under the Request Detail drop-down menu, select MSN-FNP Practicum Application and click Continue.
7. Fill in all of the information in the blank fields and click Submit. a. Please also attach your preceptor's resume or CV, license and advanced practice certification (if applicable). Students must identify a preceptor for the five practicum courses before beginning the first practicum course. Although having the same preceptor for all practicum courses is not required, it is highly encouraged as this promotes continuity of learning and the development of a professional and collegial relationship for the student. The requirements for a preceptor are listed in the MSN FNP Practicum Handbook. Once your application is submitted, you will be contacted by a Practicum Coordinator. Your Practicum Coordinator will assist you with confirming your preceptors, gaining agreements with sites (which can take some time), and making sure everything is in place for you to complete your practicum experience. You can direct practicum specific inquiries to your assigned Practicum Coordinator, or email them to
[email protected]. Please include your D# when emailing your Practicum Coordinator. Including your D# in all of your emails will help your Practicum Coordinator assist you more efficiently.
