Millennium PowerChart – PowerPlans Reference Guide Created by Organizational Learning & Development, Clinical IT/Nursing Informatics: June 14, 2013 Benefits of PowerPlans A PowerPlan is a WellSpan-defined group of orderable items. WellSpan defines groups by discipline or by function. Examples: Cardiology, Endocrinology, or Admission. Benefits: • Standardized orderable items add a level of safety. • Evidence and supporting documents are available for some orderable items. • Providers and clinicians can manage related orderables as a single entity rather than as individual orderables. • Providers and clinicians can indicate favorite PowerPlans, saving look up time for frequently used PowerPlans. Identify PowerPlan Status Changes • Planned Pending briefly displays while the user is in process of selecting and modifying a PowerPlan. It indicates the provider has selected the PowerPlan, but has not signed it. • Planned indicates the provider has signed the PowerPlan, but not initiated (activated) it. This is useful when the provider wants to plan orders in advance of initiating them. EX: Plan orders on a direct admission while the patient is still in provider’s office. A provider may customize and save the PowerPlan in a planned state multiple times prior to initiating it. • Initiated Pending briefly displays while the user is in the process of initiating (activating) a PowerPlan. It indicates the provider or clinician has clicked initiate, but has not signed the PowerPlan. • Initiated indicates the provider or clinician has initiated (activated) and signed the PowerPlan. The PowerPlan orders are part of the patient record. The ancillary departments receive the orders, and appropriate orders display on the clinician task list.
Identify PowerPlan Status Changes (cont’d.) • Completed indicates the stop date/time has been reached, or all the components have reached an end date. • Discontinued indicates a clinician or provider has discontinued an active PowerPlan, or a provider has discharged the patient. • Void indicates a clinician or provider voided a planned PowerPlan chosen in error. PowerPlan Icons • Yellow icon indicates a PowerPlan. • Double yellow icon indicates a sub-phase; a PowerPlan nested within another PowerPlan. indicates an orderable item.

Add a PowerPlan 1. Click Add on Orders tab. 2. Type name of PowerPlan, or first few characters, in Find box. OR Click appropriate folder. 3. Click appropriate PowerPlan . 4. Click Done. 5. Click checkbox on items to include in PowerPlan. OR Click checkmark to exclude a pre-selected item. NOTE: Some items may be checked and dithered (grayed out). These items are not removable. Click Return to to return to PowerPlan. 7. Click down arrow left of details column to select appropriate order sentence if needed. OR Right-click checked order and select Modify. 8. Enter or modify details as needed. IMPORTANT: Some orderable items have a Special Instructions detail. Pharmacy does not see information entered in Special Instructions. 9. Use Order Comments to communicate with Pharmacy if needed. NOTE: Click up-arrow

on Details window to open/close details. Add an Order to a PowerPlan If a needed order is not included in the standard PowerPlan, the ordering provider can add it. 1. Click Add to Phase . 2. Select Add Order. 3. Type name of order in Find box. OR Click appropriate folder. 4. Select appropriate order sentence as needed. 5. Click OK. 6. Click Done. 7. Enter details as needed. NOTE: This process adds the order to the PowerPlan for this patient only; it does not add the order to subsequent uses of the PowerPlan.
Use Merge View Use Merge View prior to initiating a PowerPlan to resolve potential duplicate orders. 1. Click Merge View . NOTE: Currently active orders display beneath yellow banners within each category of orderable items . 2. If there are duplicate orders: Remove checkmark from orderable item in planned PowerPlan. OR a. Click checkmark on active order. b. Verify discontinue reason; modify if needed. NOTE: Discontinue reason defaults to Provider Request/Order. 3. Make other modifications as needed. Intitate or Plan a PowerPlan Decide whether to initiate (make plan active) or leave in a planned state to be activated later. Initiate a PowerPlan . 1. Click Initiate NOTE: If alert(s) display, click Remove New Order OR a. Select Override Reason b. Click Continue.

Click Orders for Signature . Review orders as appropriate. Click Sign. Click Refresh. NOTE: PowerPlan status changes to Initiated in View window. IMPORTANT: ED PowerPlans auto-initiate. Use the Sign button to initiate (activate) these orders. To put an Emergency Department PowerPlan into a planned state, right-click the plan within the View window and select Plan for Later. The plan temporarily moves to a planned pending status; it moves to planned status when signed.

Review orders as appropriate. 5. Click Sign. 6. Click Refresh. Initiate a Planned PowerPlan 1. Right-click planned PowerPlan on View menu. 2. Click Initiate from shortcut menu. 3. Address alerts if needed. 4. Click Orders for Signature. 5. Review orders. 6. Click Sign. 7. Click Refresh. NOTE: PowerPlan status changes to Initiated. Use options on PowerPlan Shortcut Menu Right-click PowerPlan on View window to activate shortcut menu. • Initiate activates a Planned PowerPlan. • Discontinue inactivates a PowerPlan. • Void removes a PowerPlan not activated, but entered in error. EX: Entered on wrong patient. • Plan Information: Provides history of dates/times PowerPlan changed status, and identifies person who made changes. Activate Order in PowerPlan after Initiating Plan EX: Blood Product Transfusion PowerPlan: The provider previously ordered RBCs for the patient. Now the provider needs to place the order to administer the blood. 1. Click appropriate PowerPlan on View. 2. Click View Excluded Components all orderable items in PowerPlan. 3. Select appropriate order. 4. Modify details as needed. 5. Click Initiate. 6. Click Refresh.
Discontinue a PowerPlan (Initiated) 1. Right-click appropriate initiated PowerPlan. 2. Click Discontinue. 3. Click checkbox in Keep column to continue an order. 4. Click OK. 5. Click Orders for Signature. 6. Review orders. 7. Click Sign. 8. Click Refresh. Add a PowerPlan to Favorites 1. Click Add from Orders tab. 2. Type name of PowerPlan, or first few characters, in Find box. OR Click appropriate folder. 3. Right-click appropriate PowerPlan. 4. Click Add to Favorites. 5. Click preferred folder, if desired. 6. Click OK. Use Favorites 1. Click Add 2. 3. 4. 5. 6.

Click Favorites . Click folder containing favorite, if necessary. Click appropriate PowerPlan. Click Done. Select and modify orderable items as needed.
