United Way of Central Indiana, Inc. (UWCI) 2017 Super Bowl Trip Sweepstakes
Official Rules Eligibility: The 2017 Super Bowl Trip Sweepstakes (the “Sweepstakes”) is open only to those who are, as of the date of entry, (1) legal residents of the fifty (50) United States or the District of Columbia; and (2) eighteen (18) years of age or older. Employees and members of the board of directors of UWCI (“Sponsor”), and each such individual's immediate family members (mother, father, sister, brother, any child, husband, wife and their respective spouses, regardless of where they reside) are not eligible to participate or win a prize. All applicable federal, state and local laws apply. Void where prohibited by law. Entry Period: The Sweepstakes entry period begins at 12:00 p.m. Eastern Daylight Time (EDT) on September 1, 2017 and ends at 11:59 p.m. Eastern Standard Time (EST) on November 30, 2017 (“Promotion Period”). How to Enter: Individuals from whom UWCI receives during the Promotion Period their name, email address AND: (1) a gift of $10.00 or more to United Way of Central Indiana, OR (2) a written or electronic pledge made to United Way of Central Indiana of $10.00 or more, OR (3) a mailed-in request to enter the Sweepstakes that (a) is printed on a postcard or separate piece of paper, (b) also includes your valid residential address, phone number with area code and date of birth, (c) has proper postage affixed and (d) is received by Sponsor at: United Way of Central Indiana, Mail-in 2017 Super Bowl Trip Sweepstakes Entry, 2955 N. Meridian Street, Suite 300, Indianapolis, IN, 46208, will be entered in a drawing for the Trip Prize. NO PURCHASE, GIFT OR PLEDGE IS NECESSARY. A PURCHASE, GIFT OR PLEDGE WILL NOT INCREASE YOUR CHANCES OF WINNING. One entry per individual. Prize: Super Bowl® trip, as arranged by the Indianapolis Colts, including round-trip airfare, hotel accommodations and tickets for two to the February 2018 Super Bowl® in Minneapolis, MN (“Trip Prize”). The approximate retail value of the Trip Prize is $8,000. All Trip Prize details are at Sponsor’s sole discretion.

Winner Selection and Notification: One (1) potential Trip Prize winner will be selected at random on or about January 12, 2018, after which potential winner will be notified by telephone and/or overnight mail and/or regular mail. Potential winner and the other user of the Trip Prize will be required to execute and return an Affidavit of Eligibility, Liability and Publicity Release (where legal) and any other documents required by Sponsor within seven (7) calendar days of date of issuance of notification. Noncompliance with any of the foregoing, with these Official Rules, the inability to contact a potential winner within a reasonable time period, or any notification being returned as nondeliverable may result in disqualification and, at Sponsor’s discretion, an alternate winner being chosen. Winner status is subject to verification of eligibility and compliance with these Official Rules. Prize Acceptance: By accepting the Trip Prize, each winner and each user of the Trip Prize agrees that 1) Sponsor and its designees may use (unless prohibited by law) winner’s/user’s name, location, image, likeness and/or voice for advertising and/or any other purposes in any media now or hereafter known throughout the world in perpetuity, without further compensation, permission or notification, and 2) Sponsor shall have no liability and will be held harmless by each winner and user for any and all actions, suits, proceedings, claims, demands, orders, damages, dues, penalties, fines, costs, liabilities, obligations, losses, expenses and fees (including, without limitation, reasonable attorneys’ fees and costs) in connection with any loss, injury or damage to me as a Sweepstakes prize winner and/or user, or any other person or entity, including, without limitation, personal injury or death to me as a Sweepstakes prize winner and/or user or any other person or damage to personal or real property, resulting in whole or in part, directly or indirectly, from my acceptance, possession, use or misuse of any prize and any activity related thereto or participation in the Sweepstakes. All costs and expenses associated with the acceptance and use of a prize, including, but not limited to, all federal, state and local taxes, are the sole responsibility of the Sweepstakes prize winner and/or user. Miscellaneous: No transfer, assignment, cash redemption, or substitution of prize (or portion thereof) by a winner is permitted; however, Sponsor reserves the right to substitute for the Trip Prize or any other prize (or applicable portion thereof) a prize of comparable or greater value, as determined by Sponsor. Entrants agree, by entering, that the Sponsor reserves the right, in its sole discretion, to modify, suspend, and/or terminate this Sweepstakes (or any portion thereof) for any reason, including should a virus, bug, non-authorized human intervention or other causes beyond Sponsor’s control, including, but not limited to, war, strikes, acts of God, events and/or conditions corrupt or impair the administration, security, fairness or proper conduct of the Sweepstakes and, in the case of termination, at its discretion, select winner(s) from those eligible entries received prior to the event requiring such termination. The odds of winning depend on the number of eligible entries received. 2
Sponsor has determined that the chance to win a prize in the above-described Sweepstakes is of ‘insubstantial’ value for IRS purposes; accordingly 100% of a participating entrant’s contribution, other than that of a winner, may be eligible for deduction on the entrant’s personal income tax return under 26 CFR § 1.170A-13 and IRC § 170(f)(8). Sponsor is not responsible for lost, incomplete, invalid, unintelligible, inaccurate, undeliverable, garbled, late, illegible, stolen, postage due, damaged, delayed or misdirected notifications of entries, all of which will be disqualified; failures or malfunctions of phones, phone lines or telephone systems or other communications or malfunctions; interrupted or unavailable network, server, cable, satellite, Internet Service Provider (ISP) or other connections; miscommunications, failed computer hardware or software or other technical failures; garbled, lost, misrouted or scrambled transmissions; any error, omission, interruption, defect or delay in any transmission or communication; traffic congestion on the Internet or for any technical problem, including but not limited to any injury or damage to any person's computer or other device related to or resulting from participation in the Sweepstakes; theft, destruction or unauthorized access to, or alteration of, gifts/pledges/entries; or human-processing error; printing, typographical or other errors or omissions in these Official Rules or in any Sweepstakes-related advertisements or other materials; or other errors or problems of any kind whether mechanical, human, technical, network, electronic; or errors which may occur in connection with the administration of the Sweepstakes, the processing of gifts/ pledges/entries, the awarding or announcement of the winners and/or prize or in any other Sweepstakes-related materials. Sponsor reserves the right to disqualify from participation in the Sweepstakes any individual who Sponsor believes has attempted to tamper with, influence, or impair the administration, security, fairness, judging or proper conduct of the Sweepstakes. CAUTION: ANY ATTEMPT TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES (INCLUDING ATTORNEYS' FEES) FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION. Winner List and Official Rules: For a copy of the winner list or Official Rules, send a self‐addressed, stamped envelope to Sponsor at the address below. The request should be for a copy of the “winner list” and/or “Official Rules.” Such requests must be received between August 24, 2017 and January 31, 2018. Law: These Official Rules are governed by and shall be interpreted in accordance with the internal laws of Indiana. By entering, each entrant, along with Sponsor, irrevocably and unconditionally consents to the exclusive jurisdiction of Indiana courts and of the United States of America located in Marion County, Indiana. If any portion of these Official Rules is found to be invalid, illegal, or unenforceable, for any reason, that portion shall be severed from the rest. Claims and causes of 3
action arising out of or connected with this Sweepstakes, or any prize awarded, shall be resolved individually, without resort to any form of class action. Claims, judgments, and awards shall be limited to actual out‐of‐pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorney’s fees or any other damages of any kind. These Official Rules supersede any and all agreements or communications of any kind between you and Sponsor or anyone associated with the Sweepstakes, and cannot be modified or waived except in writing and unless properly signed by an authorized representative of Sponsor.
