Registering for myEMHShealth Revised: 11/29/2017

IMPORTANT: If at any time during your registration process you have questions, please contact our support line, which is available to you 24 hours a day, 7 days a week, 365 days a year. Call 1-877-621-8014. After signing up for myEMHShealth at your provider’s office, you will receive an email from EMHS with an invitation to join. If the e-mail does not show in your inbox, be sure to check your Spam or Junk mail folders.

The invitation will provide you with an access link to myEMHShealth along with general information about myEMHShealth. NOTE: To download this manual for step by step registration instructions, click the link in step 3 Click here to download step by step instructions To complete the registration process, follow the steps below. 1. 2. If you are registering to manage your health information a. Click I’m … OR 2. If you are registering to manage a family member’s health information a. Click I manage … 3. Click Continue

4. Verify your identity or the identity of your family member a. Enter your or your family member’s date of birth b. Enter the answer to the security question chosen while signing up c. Check the box I agree to the EMHS Terms of Use and Privacy Policy d. Click Next, Create Your Account If you have already created an account for yourself and are adding a family member go to step 6.

5. Complete all fields to create your Cerner Health account

a. Click on the blue links to review the Terms of Use and Privacy Policy b. Check the box I agree to the Cerner Health Terms of Use and Privacy Policy c. Click Create Account If you are registering to manage someone else’s account go to step 7. If not, go to step 10. Page 3 of 7

You may see the screen to the right if you have already created an account with this email address… 6. Log in to myEMHShealth a. Enter your Email address or username b. Enter your Password c. Click Sign In

If you are registering to manage someone else’s account proceed to step 7. If not, go to step 10

7. If you are managing the care of a person who is 18 years or older you will need to confirm that you have authority or permission to manage their medical record. This permission is given when your family member enrolled in the office and signed the Portal Proxy Permission Form, or if you are the legal Power of Attorney. a. Check the box I have legal authority to manage …’s health information b. Click Continue

8. Click radio button next to your family member’s name 9. Click Continue

10. You will be logged into myEMHShealth and will see some of your or your family member’s health information

11. Click one of the Return to myEMHShealth links, where you can select the provider, practice, or hospital you would like to view more of your health information for or choose the provider you want to communicate with

12. From the myEMHShealth homepage you can search for your provider to view your health information, communicate with members of your healthcare team, request medication refills and request or cancel appointments with your provider. Please note that when you search for your provider & click GO the system may take a few minutes to process your request. 13. If you do not see a provider within EMHS, you can also choose to view your hospital record.

14. On the myEMHShealth homepage, enter the name of the provider and click Go

15. Click Edit _____’s Account

16. Click My Family

17. Click Add family member

18. Choose Relationship to you 19. Click Next

20. Enter your family member’s Name, and Medical Record Number a. Add leading zeros to make the medical record number 12-digits long. NOTE: You should have received this information from provider’s office when signing up to manage your family member. 21. Click Verify

You will receive a confirmation that you have successfully added a family member to your account

To return to the myEMHShealth homepage • Click Online Services • Click Return to myEMHShealth homepage
