NABH Scoring & Grading Parameter for New hospital

The Empanelment process for hospital under Rajiv Gandhi Jeevandayee Arogya Yojana is as follows: Steps for empanelment:       

STEP I: Complete Online application form STEP II: NABH audit (by Officials from Directorate of Health services & Society) STEP III: Infrastructure Audit (by TPA and District Coordinator) STEP IV: Grading and scoring by (TPA, Society & NIC representative) STEP V: Presenting the hospitals in Empanelment committee (EC) STEP VI: in Empanelment committee (EC) decision based on all the reports by field team and application form STEP VII: If hospital is cleared, then Communication to hospital regarding score and grades

2. Criteria based on scores of 8 sheets: NABH scoring and grading criterion is as follows

Grades Criteria Minimum mandatory fulfilled in all 8 (HR, FAC, ICM, MED, EMR, TIP, SOP, PSI) sheets and marks more than 50% in each sheet. Please check the attached sheet to know the mandatory parameters Marks more than 50% in 6 sheets i.e. HR,FAC,MED,EMR,ICM,TIP Marks more than 50% in 4 sheets i.e. HR,FAC,MED,TIP Marks more than 50% in 2 sheets i.e. HR,FAC  NE If hospital scores less than 50 % marks in HR & FAC

NOTE: Total marks obtained in 8 sheets (HR, FAC, ICM, MED, EMR, TIP, SOP, PSI) /80 * 100

A, B & C grades are scores   
