Alegria  provides  a  residential  care  facility  for  the   chronically  ill,  while  also  caring  for  younger  family   members  through  its  child  development  center;  must  be   referred  by  a  case  manager  or  therapist.  They  do  not   accept  single  individuals.  Families  and  couples  ONLY.   This  is  a  24-­‐month  Transitional  Residential  Care  Facility   site  for  homeless  adults  age  18  and  older  living  with   HIV/AIDS  in  Los  Angeles  County.  Clients  must  have  a   Karnofsky  score  of  70  or  higher  and  a  Global  Assessment   Functioning  scale  of  65  or  less.  Sliding  Scale   Agency  referral  letter  from  case  manager  with  general   demographics;  source  of  income,  housing  and  medical   history.  Contact  Rosa  for  appointment.  HIV/AIDS   diagnosed  form  required.  Diagnosis  form  should  not  be   older  than  6  months.   Residential  treatment  for  men  and  women  living  with   HIV/AIDS  in  Southern  California.  Drug  testing,  TB  Test.   Must  complete  the  SERRA  waitlist  and  group  home   application.   Alexandria  House  provides  services  for  women  and   children  ONLY  .Must  call  first  for  intake;  second  will  be  a   face-­‐to-­‐face  interview.  Admission  for  the  program  is  first   come  first  served.  Alexandria  house  is  not  an  emergency   shelter  and  is  unable  to  offer  housing  on  an  immediate  or   walk-­‐in  basis.  Tuberculosis  test  required,  proof  of  income,   and  proof  of  Identification.   Transitional  living  program,  a  youth  takes  the  initials  steps   in  living  independently.  To  qualify  for  the  program,  a  youth   must  be  in  school  or  working  full  time.  If  working,  they  are   required  to  put  a  portion  of  their  wages  into  savings.  For   youth  18-­‐23years  old.   The  goal  is  that  at  the  end  of  her  stay  at  Farley  House,  a   woman  is  well  prepared  for  maintaining  employment,   financial  stability,  and  permanent  housing.  Provides  a  21-­‐ one  bedroom  apartment  for  mothers  and  their  children   and  single  women.   Residents  are  required  to  have  an  income  prior  to  moving   in  and  pay  a  modest  percentage  for  rent.  Those  women   who  are  not  employed  must  seek  employment  and  are   encouraged  to  further  their  education  with  the  goal  that   they  will  find  full-­‐time  work.    Provides  30  homeless   women  with  private,  furnished  rooms.   The  PATHCares  program  offers  support  to  me  with   HIV/AIDS  and  those  with  other  special  health  needs.  The   program  has  18  private  rooms  and  connects  with  clients   with  all  of  the  services  available  in  the  PATHMall  enabling   them  to  achieve  greater  health  and  self  self-­‐suffiency   The  center  provides  20  beds  of  transitional  housing  and   services  for  single  mothers  and  their  children  in  the  Los   Angeles  area.    No  fee  for  housing  but  will  required  saving  a   percentage  of  income.  60  days  of  sobriety  and  must   participate  in  an  intensive  6  month  case  management     No  fee  for  housing  but  will  be  required  to  save  a   percentage  of  income.  RHC  provides  a  total  of  39   transitional  housing  beds  and  support  for  homeless  men   with  a  clean  bed  and  space  for  personal  belongings,  and  is   able  to  stay  for  up  to  6  months  as  he  rebuilds  his  life.

The  program  provides  a  unique  support  system  that  is   catered  towards  the  specific  needs  of  homeless  women.   Female  participants  partake  in  special  workshops  that  deal   with  topics  such  as  self-­‐esteem,  anger  management,  and   personal  finances.  No  fees  for  housing,  but  will  be   required  to  save  a  percentage  of  income.   Provides  youth  with  up  to  8  months  of  housing.  Youth   enrolled  in  transitional  housing  are  typically  going  to   school  or  work  on  a  daily  basis.   18-­‐24  years  of  age.  MALES  ONLY   Transitional  living  program  offers  you  a  free,  welcoming   place  to  stay  for  up  to  18  months  while  developing  the   skills,  resources.  Staff  will  help  you  find  and  maintain  a  job,   enroll  in  school,  and  manage  your  savings  so  you  can   succeed.     Homeless  adult  individuals,  with  a  priority  on  the  elderly   and  veterans,  are  provided  board  and  lodging  at  FASGI's   16-­‐bed  shelter.  Goals  of  placing  the  client  in  permanent   housing.   Assistance  finding  housing  and  first  month’s  rent,  help   with  filling  paperwork  for  government,  educational  and   vocational  programs,  as   Well   as   assistance   with   maintaining   contact   with   the   parole   board   and   the   stipulations   of   their   parole.   MEN   ONLY,   Must   attend   orientation   on   the   1st   Monday   of   the   month  at  1pm   The  Transitional  Housing  Program  provides  a  safe  and   stable  environment  for  families  with  children  while   providing  support  and  opportunities  to  attain  their  goals   that  will  lead  to  housing.  

Los  Angeles  House  of  Ruth  serves  homeless  and   battered  women  and  their  children  ONLY.  Clients   include  pregnant  women,  large  fatherless  families,  non-­‐ English  speaking  mothers,  and  women  who  are  not   eligible  for  emergency  human  services.  FOR   EMERGENCY  SHELTER  CALL  211   For  youth’s  ages  10-­‐17.  Runaway  teens,  most  of  which  are   fleeing  abusive  families,  find  refuge  at  Angel’s  Flight  near   Hollywood.  Substance  abuse  treatment  and  recovery,   mental  health  counseling,  food  and  toiletries,  job   search  guidance,  gang  prevention  and  intervention,   tutoring,  art,  dance  and  music  instruction  are  central   program  services.   Temporary  Housing  $175  a  week  or  $750.00  per  month   that  will  include  3  meals  per  day  Monday  –  Saturday,  it  will   also  include  all  utilities.  Must  be  independent,  required   income  $1,400  per  month.  No  fee  for  application.  NO   criminal  background  will  be  accepted.  $350.00  deposit  to   move  in.   24-­‐month  program  for  mothers  and  children  living  with   HIV  and  AIDS.  Drug  testing,  TB-­‐test  required.  Must   complete  the  SERRA  waitlist  and  group  home  applications.

Substance  Abuse  Foundation  provides  Veterans  who    are   in  need  of  transitional  housing  and  are  dual,  and  triple-­‐ diagnosed  with  co-­‐existing  substance  abuse,    mental   health  and  medical  diagnoses  with  quality  housing  and   services  that  meet  the  needs  of  this  special    population.   Veterans  are  referred  through  the  VA  Greater  Los  Angeles   Healthcare  System,  Health  Care  for    Homeless  Veterans   Program,  which  provides  case  management  services  to   Veterans  and  facilitates  their    access  to  a  broad  range  of   medical,  mental  health  and  rehabilitative  services.   This  residential  treatment  program  provides  up  to  six   months  of  community-­‐based  substance  abuse  treatment   and  supportive  services  to  eligible  low-­‐income  female   offenders.  It  is  designed  to  help  women  recover  from   addiction  and  to  prevent  recidivism.  Participants  are   referred  directly  from  the  drug  treatment  program  at  the   California  Institution  for  Women  (CIW)  or  by  their  parole.   This  program  serves  non-­‐violent,  low-­‐income  male  and   female  drug  offenders  who  parole  in  Los  Angeles  County.   A  full  continuum  of  care,  from  intensive  residential   treatment  to  outpatient  treatment  in  varying  levels  of   intensity  is  provided,  as  well  as  Alcohol  and  Drug  Free   Living  Center  housing.  Eligible  participants  must  be   referred  by  their  parole  agent  of  record  (at  the  Santa  Fe   Springs,  Huntington  Park  or  Long  Beach/Harbor  Parole   Complexes)  in  Los  Angeles  County.   This  is  a  24-­‐month  Transitional  Residential  Care  Facility   site  for  homeless  adults  age  18  and  older  living  with   HIV/AIDS  in  Los  Angeles  County.  Clients  must  have  a   Karnofsky  score  of  70  or  higher  and  a  Global  Assessment   Functioning  scale  of  65  or  less.  Sliding  Scale   This  is  a  24-­‐month  Transitional  Residential  Care  Facility   site  for  homeless  adults  age  18  and  older  living  with   HIV/AIDS  in  Los  Angeles  County.  Clients  must  have  a   Karnofsky  score  of  70  or  higher  and  a  Global  Assessment   Functioning  scale  of  65  or  less.  Sliding  Scale,  must  be   physically  able  to  work.
