AUDITS [email protected] We are responsible for ordering all physical and voluntary audits. No audit is necessary on policies with auditable premium less than $ 5,000 unless you or we determine that the policy should be audited. Physical Audits 1. Physical Audits are required on all policies with auditable premium in excess of $ 5,000. 2. We will order audits from our office. We will also process and issue the necessary premium audit adjustment endorsement, if necessary, after we receive the Physical Audit report. Uncollectible Audit Procedure Once an audit endorsement is received by you, suspense it for payment and follow up in 15 days if not paid. If, after 45 days, but no more than 60 days from the date an endorsement is received and you have been unable to collect the premium, you may then return it to Century to take over the audit for direct collection. If you have not returned the audit to Century for direct collection and we have not received payment from you within 60 days after the audit endorsement is mailed to you, the Audit Department will follow up with you by fax or mail to request payment within the next 10 days. If the Audit Department does not receive a response from you within 10 days, we will take over the audit for direct collection. Once Century has taken over an audit for direct collection, no commission is payable on the audit, unless you are subsequently able to collect (through your efforts) the audit, in which case, commission is payable at 50% of the original commission rate on the policy. Century reserves the right to determine if successful collection was “through your efforts”.

BROKER OF RECORD LETTERS There are two types of Broker of Record letters we will accept: 1. Insured’s letter (on his/her letterhead) as follows: a. Lists all Century policies by number and show expiration dates. b. Letter is dated and states that letter applies to the renewal of the referenced policy. c. Lists the retail broker and the new general agent (GA). d. Letter is addressed to issuing carrier (can be in care of the new GA). e. We (Century) receive it at least seven (7) working days prior to renewal date (so we can notify incumbent GA of the change). 2. Incumbent retail producer’s letter (on his/her letterhead) as follows: a. Lists all Century policies by number and shows expiration date(s). b. Letter is dated and states that letter applies to the renewal of the referenced policy. c. Confirms that he/she is the current broker writing this particular policy through (name of current GA) and now wishes to place the renewal through (name of new GA). d. Letter is addressed to issuing carrier (can be in care of the new GA). e. We receive it at least seven (7) working days prior to renewal date (so we can notify incumbent GA of the change). We will not normally accept a mid-term broker of record letter. If you have an unusual situation that justifies such a change, then submit full details to us and we will review and advise you.

CERTIFICATES OF INSURANCE While we prefer that our General Agents issue certificates of insurance, we are not opposed to allowing you to delegate this responsibility to your sub-producers under the following conditions: Only Acord Certificates are used. If you need to issue a certificate other than an Acord, you must submit it for Company approval. No terms or conditions of the policy are changed by the certificate. All such certificates must be sent to you and reviewed by the appropriate person in your office. If you receive a certificate not issued in accordance with these instructions, you must send written instructions to your sub-producer outlining the proper procedures to follow and advising them to reissue the certificate correctly and send a copy to you. All changes to policy terms and conditions, including, but not limited to additional Insureds, waiver of subrogation, mandatory NOC, etc., must be endorsed. The General Agent (you) will be fully responsible and will indemnify Century for any and all errors and/or omissions contained within any Certificate and for your sub-producers’ failure to issue any Certificate in full accordance with the guidelines herein. CLAIMS [email protected] Please report all accidents or events to us immediately that is likely to give rise to a claim. Claims should be reported to the Century Home Office in Westerville, Ohio by one of the following methods: If the loss notice only contains loss information with no attachments, the best avenue is to report it via the Internet. Our web address is www.centurysurety.com. Once on the site, click the “Report a Claim” tab and follow its ensuing directions. If the loss notice does contain additional information such as limited attachments, then it is advisable to use the fax reporting at: Eastern Regional Office: 614-895-7040. If the loss notice and attachments are longer than approximately 20 pages, it is advisable to “Overnight” the entire package to our claims department. Only in rare instances should claims be reported via the telephone at 800-878-7389; and then only if that reporting is followed up in a written format noting that the claim was already reported and on what date. Summons and Complaints Handling Initially, an attempt should be made to determine “who” was served, “when” they were served, and “how” the lawsuit was delivered. However, do NOT hold up the transmittal of a lawsuit because of a lack of this information. If this information cannot be obtained the same day you receive the lawsuit, forward the lawsuit to us immediately! If the lawsuit is approximately 20 pages or less, please fax it to us at 614-895-7040. If it is significantly longer, then overnight it to us. You can then supplement the information on “who”, “when”, and “how” to us later. We suggested that a representative of your office follow the fax / overnight mailing of the lawsuit, with a phone call to our claims department at 800-878-7389. This will ensure that we have received it and help us avoid any potential problems in protecting the insured’s interests. When sending a lawsuit to us, please be sure to advise, when it was received in your office. Do NOT send these by regular mail, unless it is just a photocopy of what you have already faxed or overnighted. Assignment of Claims The Claims Department at Century makes all assignments to independent adjusters and defense attorneys. When being advised of a loss, please ask the insured to take necessary measures or assist in making temporary repairs to mitigate further loss. Claims opening and closing notices are provided to the general agent. If you need to check on the status of a claim, please direct your inquires to the assigned claims examiner which will be noted on the opening notice. Our general claims number is 800-878-7389. All claims checks are issued by Century and this authority is not delegated to any agent.

DEALERS PHYSICAL DAMAGE Dealers Physical Damage covers owned autos, dealer’s inventory and consigned autos. 100% coinsurance applies and Dealers Physical Damage must be insured to value to avoid a penalty. Mono-line coverage is available. Consigned Autos held for sale, are to be insured under Dealers Physical Damage. Earthquake, Mudslide and Flood are excluded in all states. Theft coverage is automatically excluded for unprotected lots. Protected Lot defined: When the described premises are closed for business: o All entrances, exits, or openings and the entire perimeter are surrounded by fences with gates or heavy chains and locks. o The building is protected by locked and secured doors, windows or other types of openings. Theft from unprotected lot buyback is available subject to the following criteria: o The lot must be located in a town with a population of less than 50,000. o Location must be at least five miles from any major metropolitan areas. o Must have adequate after hours lighting. o No prior theft losses. The rate for this buyback endorsement is a 10% surcharge on the Specified Causes of Loss or Comprehensive rate. DRIVER ELIGIBILITY REQUIREMENTS Drivers must meet the criteria listed below to be eligible for our program. If they do not meet these guidelines, they must be excluded from any driving exposure covered by our policy. 1. Must be at least 18 years old. 2. Must have a valid United States Drivers License. 3. Individual drivers with violations listed as Submit or those with a driver’s surcharge over 1.60 require company underwriter approval. 4. Personal use is available to Owners, Partners, Members and their spouses; and current Employees. These drivers must have at least 5 years driving experience as confirmed by their MVR. All drivers with personal use must be individually listed on the policy. 5. Liability limits for any driver under the age of 21 will be reduced to minimum statutory limits. 6. “Owners” and “Partners” of an individual proprietorship or partnership (active or inactive) CANNOT be excluded, therefore, the risk must be declined or cancellation notice sent. 7. If the policy is a corporation, unacceptable officers or members may be excluded.

DRIVER HISTORY / MOTOR VEHICLE REPORT REQUIREMENTS [email protected] Your office is responsible for obtaining MVR’s and forwarding to Century within 30 days of binding or with the addition of new employees during the policy term. MVR’s are required for only those classes as noted on the Classification table and on risks with scheduled autos. MVR’s (with 3 years information) are required for ALL owners, employees and drivers for new business unless indicated otherwise on the classification table. MVR’s must include at minimum: Date of Birth, Legal Name, and Violations for the past 3 years, Expiration Date, Class of License, Status of License, and the MVR issuance date. DRIVER SURCHARGE The driver surcharge applies only when MVRs are required, as noted on the Classification table and on risks with scheduled autos. The average driver surcharge is established at policy or renewal inception and does not require mid-term adjustment. The driver surcharge is figured by starting with a factor of 1.00 per person and adding the applicable increment shown below for each violation. Total the individual developed surcharges and divide by the number of drivers, to arrive at the policy level Driver Surcharge. Do not include excluded drivers in the calculation. Example: if the individual driver surcharges total to 3.40 and you have 3 drivers the calculation would be 3.40 ÷ 3 drivers = 1.13 policy level driver surcharge that would be entered into the rating system. Rating worksheets must be included with all Additional Premium / Return Premium endorsements. FEES (Policy, Inspection, Broker, Etc.) Fees may be applied to each policy subject to state regulations and you are responsible for applying all applicable fees and taxes to your quotes.

FILE DOCUMENTS It is the agent’s responsibility to obtain and forward all documents in a timely manner. Failure to provide required underwriting documents will result in cancellation of the policy. All file documents should be sent to [email protected] The following are required within 30 Days of binding: 1. Signed Garage Application (and Supplemental when applicable). 2. A minimum of 3 Years Prior Carrier Information - required on all accounts. A minimum of 3 Years Hard Copy currently valued Loss Runs - required if premium exceeds $5,000.

3. MVR’s (currently valued, when applicable). 4. UM, UIM or PIP Rejection/Selection Forms (when applicable). 5. Inspection – refer to inspection requirements. FILINGS Any risk requiring Federal Filings is prohibited. All State Filings are to be done by your office, and copies provided to the Company. Your office is responsible for cancelling of all State Filings - mid-term cancellation and policy expiration.

GARAGEKEEPERS COVERAGE Garagekeepers coverage is for Customer’s Autos in the risks care, custody and control, and does not cover dealer’s inventory. When offering Garagekeepers coverage, it is imperative that the lot limit be reflective of the total exposed values, as the Garage Coverage Form and Auto Dealers Coverage Form do not apply a “coinsurance” penalty. We suggest a minimum valuation of $5,000 per auto or parking space when calculating the correct Garagekeepers limit Mono-line GKL coverage is available. Consigned Autos held for sale, are to be insured under Dealers Physical Damage. Earthquake, Mudslide and Flood are excluded in all states. Theft coverage is automatically excluded for unprotected lots. INCIDENTAL GENERAL LIABILITY CLASSES The following General Liability classes can be added as an incidental exposure (less than 20% of total operations) when they occur in conjunction with Auto Dealers or Garage Operations for no additional premium. If the exposure exceeds 20% of total operations, additional rating is required. Inspections, except in remote areas, should be completed within approximately four (4) weeks. Inspections should cover all of the normal underwriting points (this can usually be done with a check list type inspection plus a half page to a full page narrative in addition to the check list). Any unusual points that need to be covered or any points that need some specific attention should be noted in the order form to the inspection company.

INSURANCE DEPARTMENT COMPLAINTS Insurance Department complaints are inevitable and must be handled as priority items. We will make every effort to handle complaints directly with the Insurance Department, but we will sometimes need you to help us to develop background information. When we call upon you, please understand we are dealing with time constraints and we will expect your prompt response. In the event that you receive contact or correspondence directly from the insurance department, or other regulatory or governmental authority, please notify us within twenty-four (24) hours.

LAPSE IN COVERAGE We will consider submissions with a lapse in coverage with a signed No-Loss letter. LICENSE PLATES Dealer plates are used by Dealer operations to move or operate owned, but unregistered vehicles in their inventory. Registration plates are used by Dealer and Non-Dealer operations to move or operate non-owned autos. Some states also refer to them as Repairer or Transporter Plates. Please offer matching limits of liability coverage when writing multiple polices for the same account. Policies packaged with Auto Dealers or Garage Coverage Form will be handled within the Garage underwriting department.

It is your responsibility to provide complete information, and we expect your prompt response to requests for missing or incomplete information. Information required for issuance: 1. SURPLUS LINES LAWS AND LICENSING You are required to comply with all applicable laws and regulations of the state(s) in which you operate as respects to the procurement and placement of insurance (including without limitation, obtaining and maintaining all necessary licenses). You shall provide the appointing authority in each state with all the required filings, affidavits and reports. You are responsible for collection and payment of all excess and surplus lines premium taxes, or other required taxes or fees that may become due to any taxing authority within the boundaries of any state or commonwealth.
