Whether you are looking to manage your stress levels, eat better or get support in reaching your health goals, RedBrick is here to provide you with the tools, resources and motivation you need to meet your health and well-being goals. • Earn up to a $600 premium reduction when you (and your spouse, if enrolled in the health plan) complete your RedBrick Compass® health assessment and a health screening, and declare to be tobacco free (or complete a tobacco cessation program with RedBrick Health). • Earn up to $50 per quarter into your HRA/HSA by having healthy or improved screening results and/or by participating in healthy activities.

WHO CAN PARTICIPATE?

RedBrick programs and rewards are available to all the employees of the subsidiaries of The Schwan Food Company. Spouses enrolled in the Company’s health plan are also eligible to participate. Schwan’s Healthy Living Allowance program will reimburse eligible employees up to $150 per year for expenses associated with specific health and wellness activities that contribute to a healthy lifestyle. Omada is a free 16-week online program, for eligible employees, which uses an innovative approach that combines behavior science, social support and technology to achieve changes that lead to better health. Grand Rounds is a free service for health plan enrolled employees & their enrolled dependents, which offers access to trusted medical care if you are facing a health issue. Whether you need a second opinion on your current diagnosis or an appointment with a qualityverified physician, Grand Rounds can help ensure you’re getting the best possible care.

You can get head-to-toe treatment at the cost of a typical co-pay and much lower than the usual cost of urgent care. LifeMatters provides you with confidential support and assistance with any of your life, work, family or well-being issues. The program’s user-friendly, confidential services are available to you and your eligible dependents 24/7/365. Healthy results

It’s important to make the most of your time right from the start. RedBrick can help you find the best activities based on your health to help you keep moving in the right direction.

If you meet the healthy measures listed below, or see improvements from last year’s results, you’ll earn $15 per quarter, per measure. Earn up to a $50 HRA/HSA deposit per quarter ($200 annually). Your maximum healthy reward can be earned by achieving the following targeted health screening measures or improvements.

Earn $15: Blood Pressure < 140/90 or a decrease of 20mm systolic or 10mm diastolic

Earn $15: Body Mass Index (BMI) Either between 18.5 and 29.9 or a decrease of 5% or more

Learn your health numbers

Participate in a health screening to get a better picture of your health. Health screenings can be completed on-site at The Schwan Food Company (if available), from a partner lab or retail clinic with a voucher, or through your health provider. DOT Physicals also qualify for the health screening.

Earn $15: Cholesterol < 219 or a decrease of 10% or more

Earn $15: Glucose < 125 (fasting) or < 140 (non-fasting)

Healthy activities Choose from the options below to participate with RedBrick.

REDBRICK TRACK® Good health is about balancing physical activity and healthy eating with things like quality sleep and finding ways to relax. RedBrick Track is designed to help you build and reinforce healthy daily habits in just a few minutes each day. You can track your progress yourself or sync other devices and apps with your account.

Complete your Tobacco Declaration

Declare your tobacco status for 2017. Not tobacco free? Complete a Tobacco Cessation coaching program with RedBrick Health or an online Be Tobacco Free RedBrick Journey STEP 5

Choose your Direction and healthy activities RedBrick will offer you personalized recommendations for healthy activities, and your HealthMap will point you in the right Direction.

Rewards In order to receive your $600 health plan premium reduction* effective January 1, 2017, both the health plan enrolled employee and spouse, if enrolled, must complete the Compass health assessment, the health screening, and declare to be tobacco free**. Each quarter, health plan enrolled employees will also have the opportunity to earn up to $50 (up to $200 annually) by completing healthy activities. • Eligible employees will have rewards deposited into their Health Savings Account (HSA) or Health Reimbursement Account (HRA). • Employees who are enrolled in a Schwan’s health plan and have premiums deducted from pay are eligible to earn rewards. • All other employees are eligible to participate and be entered into quarterly drawings for prizes.

Important dates for participation DECEMBER 20, 2016 Deadline to complete the health assessment, health screening and declare to be tobacco free for premium reductions to take effect on January 1, 2017. Paper forms must be faxed or postmarked by December 10, 2016.

Journeys are made up of steps that inspire new, healthy actions. Try an online Journey to be more active, stress less, eat healthier, lose weight and more. You get to pick the healthy, inspiring steps that appeal to you, and unlock new levels of healthy habits as you go. It’s all up to you.

HEALTH COACHING With a health coach, you’ll benefit from working directly with a certified expert such as a registered dietitian, exercise physiologist or registered nurse. Our coaches work with you over the phone to answer your health questions, provide support and set goals.

CHALLENGES Join a team of co-workers or create your own team of one with fun and social challenges. It’s your chance to fire up your competitive spirit, boost your motivation and stretch beyond your usual routine.
