April-May, 2015- High School Coaching Staff will have sports sign-up meetings for current high school students. Please listen for announcements! May 1, 2015- PAY FOR IT site becomes active for the 2015-2016 school year. (July 15, 2015 becomes active for incoming Deerfield Middle School students) May, 2015- All current Governor Livingston High School students Sports Registration Packets are due to the High School Nurse’s Office. May, 2015- In house sports physicals for Columbia Middle School students (current 8th graders) conducted during CMS school day by Care Station June, 2015- In house sports physicals for Deerfield Middle School students (current 8th graders) conducted at Governor Livingston High School by Care Station Fall Sport Practice Start Dates: No student will be permitted to practice without a full medical clearance from our health office staff (Nurse and Athletic Trainer). It is your responsibility to complete the forms in a timely fashion and to adhere to deadlines. Please contact your coaches for additional information about practice/try-outs and summer team events. Instructions for Sports Registration for the 2015/2016 School Year: Information for any student interested in participating in sports during the 2015/ 2016 school year: Review all forms posted on website, print pages 1 through 11 only and complete. The remaining pages are for your reference. ***In addition, if your child requires any medication while participating in sports (inhaler, epipen, insulin/ glucagon), please review the District Medication Policy, print the Medication Authorization Form and submit the completed forms prior to the start of preseason. *** All current Governor Livingston HS students’ registration packets are to be turned into the Governor Livingston Health Office by Friday, May 1st. All current Columbia MS students’ registration packets are to be turned into the Columbia Health Office by Monday, May 18th. The school physician will be at GL in May (Dates: TBA) and at Columbia on May (Dates: TBA) to do sports physicals for any athlete who would like their sports physical completed in school. Highly suggested! PLEASE NOTE: the Pay‐For‐It site for the 2015/2016 school year will NOT be available until May 1st for current Berkeley Heights residents and after July 15th for Mountainside Residents If you have any questions, please don’t hesitate to call the Health Office.
