IDA JEAN ORLANDO USE OF METAPARADIGM CONCEPTS IN THEORY Jillian Kohl Patricia Derenzy Christie Kleinert Larry Horchner Rachel Fox

INTRODUCTION Ida Jean Orlando, RN, Nursing Theorist  Born 1926

Nursing Education  New York Medical College

Focus:  The dynamic nurse-patient relationship

Major Publication: (1961)  The dynamic nurse-patient relationship: Function process and principals

METAPARADIGM CONCEPTS AS DEFINED BY ORLANDO  PERSON  Each patient is unique and individual in how they respond

 HEALTH  State free of physical and metal stress  Sense of well-being

 The patient-nurse relationship is dynamic and needs change as illness progress or improves  The actions and reactions of both the patient and nurse, influence the nurse-patient relationship

 Always varying on a continuum  Behaviors, attitudes and beliefs of patient play a role in state of health

METAPARADIGM CONCEPTS AS DEFINED BY ORLANDO  ENVIRONMENT  Involves factors beyond nurse control (other patients, necessary equipment, hospital protocols, presence of family members)  Nurse uses patient response to these factors to create a perception of patient needs  Nurse should assess the environment and seek external resources to make changes that can lessen patient stress

 NURSE  Uses the nursing process to discover patient needs and their response to nursing interventions  A profession that is constantly evolving based on nurse-patient responses  Consider physical and mental needs  Treat holistically

Application of Ida Jean Orlando’s theory to the improvement of contemporary health care and application to evidence based practice.

Local  In order to read patients and predict behaviors you need constant interaction (Nursing Theories, 2013)  This forces nurses to see their patient as the individual (Nursing Process, 2014)  Does the individual have access to grocery stores? Farmer’s Markets?  Does the individual live in a safe neighborhood conducive to a healthy living environment?  Facilitation of community and healthcare resources

Regional  We need to treat the whole before we can treat the individual  Cultural diversity awareness and education  Public transportation  Appropriately staffed health care settings

Global  We can look at the population as a whole and create a plan which will meet the needs of a majority of the population  Globally we can see concern for the planet, seeking to make the world a better place for those that follow  Access to food/clean water  Immunization  Disease epidemics

Evidence Based Practice  Nurses should not choose things for their patients, but educate them and support their decisions  Researchers must remember that each individual has their own set of moral values and beliefs molded by their environment as well as individual perceptions  Measuring outcomes
